The dog that real estate magnate Leona Helmsley bequeathed $12 million has died, the family trust says, according to The Daily News.

In fact, says the trust, the 12-year-old Maltese, named Trouble, died Dec. 13 and was cremated.

Helmsley, who died in 2007 at 87, bequeathed Trouble $12 million, but a judge trimmed the bequest to $2 million.

The manager of a Helmsley hotel in Florida, who looked after the dog, said at the time that he could take care of Trouble's needs on $100,000 a year: $8,000 for grooming, $1,200 for food and the rest for his fee and a full-time security guard, the News says.

Helmsley was the widow of Harry Helmsley and helped run their hotel and real estate empire.

Dubbed the "Queen of Mean" in the media, Helmsley served 21 months in prison for evading federal taxes by billing her businesses for mansion renovations.

Trust spokeswoman Eileen Sullivan would not say how much was left of Trouble's money, but what remained went into the trust.

Senate votes to let Fed lower debit card fees

The Senate voted today to let the Federal Reserve lower debit card fees as merchants triumph over banks.

At issue is a Fed proposal that's set to take effect next month and would cap the fees at 12 cents per transaction, compared with the current average of 44 cents per swipe.

Senators who wanted to block the Fed's plan needed 60 votes to prevail, but came up six short at 54-45.

The Fed says the current transaction fee earns banks and credit card companies $16 billion a year.

Stores say the lower swipe fees will let them lower their prices. Banks say they will have to raise other fees, such as checking account fees.

Recipe: Harvest pumpkin soup

This soup recipe is sure to warm anyone up when the air turns crisp.

Ingredients

?2 tbsp unsalted butter

?1 medium onion, minced

?2 medium garlic cloves, minced or pressed through a garlic press (about 2 tsp)

?½ tsp ground cumin

?½ tsp ground coriander

?¼ tsp ground nutmeg

?3 cups low-sodium chicken broth, plus extra as needed

?2 cups water

?1 (15-oz) can pure pumpkin (not pumpkin-pie filling)

?¼ cup maple syrup

?½ cup half-and-half

?Salt and ground black pepper

Directions

Melt the butter in a large Dutch oven over medium heat. Add the onion and cook until softened, 5 to 7 minutes. Stir in the garlic, cumin, coriander and nutmeg and cook until fragrant, about 30 seconds.

Stir in the broth, water, pumpkin and maple syrup, scraping up any browned bits, and bring to a boil. Reduce to a simmer and cook until the flavors have melded, about 15 minutes.

Working in batches, purée the soup until smooth, 1 to 2 minutes. Return the soup to a clean pot. Stir in the half-and-half and additional broth as needed to adjust the soup's consistency. Heat the soup gently over low heat until hot (do not boil). Season with salt and pepper to taste and serve.

Servings:

4 to 6 generous one-cup servings.

Nutrition per serving: 140 calories; 7 grams of fat; 4 grams of saturated fat; 3 grams of protein; 18 grams of carbohydrates; 4 grams of fiber;15 milligrams of cholesterol; 300 milligrams of sodium.

What is a hand-off? It’s when a patient transfers from one setting of care to another. Common examples include:

nursing home to hospital emergency room (and back again)

hospital to rehabilitation facility

hospital to hospice care

Thousands upon thousands of patient transfers occur every day. In fact, an article in the Wall Street Journal last week quoted the President of the organization that accredits hospitals and other health care facilities (The Joint Commission) as stating:

There are 4,000 hand-offs a day in a typical teaching hospital. If 90% go flawlessly, that’s still 400 failures per day.

The Joint Commission collaborated with 10 hospitals and systems that volunteered to participate in a project led by the Commission and found that transfers were defective 37% of the time. So if I do the math here, a teaching hospital that has 4,000 hand-offs a DAY would be within the average to have 1480 flawed communications PER DAY when transferring care in to, or out of, the hospital setting. Houston, we have a problem!

Within the Journal article is a link to the chart from the Commissions’ Report that documents the validated root causes for hand-off communication failures. (The chart is also available through The Joint Commission Site on page 6 of the document). There are 4 causes (out of 20) that were identified that jumped out at me. Three of the 4 were reported by 7 of the 10 participating hospitals:

Culture does not promote successful hand-off, e.g. lack of teamwork and respect

Expectations between sender and receiver differ

Sender, who has little knowledge of patient, is handing off patient to receiver

The 4th one was validated at every one of the 10 hospitals. These are not small, community facilities…they include Johns Hopkins, Mayo Clinic, NY Presbyterian, and Massachusetts General. The most common cause of hand-off failure was:

Sender provides inaccurate or incomplete information, e.g. medication list, DNR [Do Not Resuscitate], concerns/issues, contact information.

I cannot say this often enough. It is critical that patients have a trusted friend or family member building a file of the patient’s care history and reviewing that care history with every new facility or physician who cares for the patient.

The good news is that organizations like The Joint Commission have started to take notice and recognize the dire need for quality controls. The bad news is that change takes time and while the incidence of medical errors will (hopefully) ultimately decrease significantly, the risk will never be totally eliminated.
